1. Direct Primary Care Membership
Comprehensive, relationship-based care for children from newborn through age 21.
What’s included:
• Annual wellness exams & preventive care
• Newborn care and early infancy support
• Same-day sick visits
• Telehealth visits when appropriate
• Mental & behavioral health visits
• Sports physicals & school paperwork
• Integrative medicine evaluations
• In-office testing (strep, flu, urinalysis)
• Select minor procedures (wart treatment, suture/staple removal)
• Lab, imaging, and specialist coordination
• Referrals to trusted specialists
Access & Experience:
• Direct access to your doctor via phone & secure text
• Longer visits (30–60 minutes)
• Little to no waiting room time
• All of the visits and communication with your physician as needed
• Transparent pricing (no surprise bills)
The following are examples of services which are not included in your membership fee:
- Vaccines – Reviewed and discussed; administered through local health department
- Lab testing -can submit to your insurance for reimbursement consideration, can use the lab of your choice. Discounted cash prices are available through the office. Functional medicine testing is usually not covered by insurance
- Imaging – check with your insurance, most insurers will reimburse for these in accordance with their regular coverage policies. Can use the facility of your choice.
- Supplements – are offered through discounted prices through Fullscript
- ER visits – directed to pediatric ER when appropriate.
- Hospital admissions – managed by hospitalist team
Additional Information
Please note that we do not accept any form of medical insurance, but you are welcome to use insurance for labs, imaging, and specialist referrals.
2. Integrative Medicine Consults
Whole-child, root-cause care designed to address complex or chronic concerns.
Ideal for:

• Chronic abdominal pain or headaches

- Advanced functional medicine testing & interpretation

- Advanced functional medicine testing fees are not covered in the membership

Newborn (0–12 months) – $220/month
• Routine well visits (after hospital discharge, 1 week old, 1, 2, 4, 6, 9, 12 months old)
• Sick visits & ongoing care

One time enrollment fee

Children (1–21 years) – $140/month
Full access to all primary care services
- well child / physical once per year – 1 hour long
- 5 additional visits per year
- Daytime call text access to your doctor for urgent issues
- Ongoing communication through the portal for non urgent issues
- Additional visits for sick or specialized care can be added
One time enrollment fee – $150- covers transfer of records, review of previous tests and records
- for multiple family members will cap membership at 300/MONTH
First 100 days package - $1500 one time fee - the best gift for your family
- All visits at your home – within 10 miles of the office
- All routine sick visits and well visits
- All routine testing – newborn screen
- Pre birth consultation – in person / video
- Stool microbiome testing
- Direct access to physician
Integrative Medicine Package – $250/month (3–6 months)
- Consult / 2nd opinion focused care for a specific problem
- Initial 60-90 minute visite
- 1/month follow up visit in person / video
– Functional medicine testing (stool microbiome, food sensitivities, genomics) – Fees foor testing not included
– Customized plan of care
– Non urgent communication through the portal
– Patients must have at least part time residence in AZ
- Does not include routine primary care or sick care visits, access to physician outside business hours
Mental health package - $250/month (3-6 months)
- Include screenings for anxiety, ADHD, depression, PANDAS
- Initial 90 min visit and follow up 1/month
- Periodic check ins
- Conventional labs (could be covered by your insurance) and functional medicine testing (self pay)
- Customized plan for supplement, diet, lifestyle
